Avalola Home Care, LLC provides supportive community living and personal care services in a clean, comfortable, secure, family-based home environment. Upon admission, a written assessment and individual service plan (ISP) identifying the resident’s needs and abilities in the areas of:
  • Activities of Daily Living (ADLs)
  • Medication Administration and Management
  • Physical and Mental Health
  • Level of Supervision Required in the Home and Community
  • Vocational Needs/Wants
  • Recreational and Social Needs/Wants
  • Transportation Needs
  • Individual Service Plan (ISP) An ISP is developed for each resident and reviewed every 6 months. In the event of significant change in a resident’s needs or wants or upon request of the resident, resident’s guardian, or other professional, the ISP will be reviewed sooner. Residents’ family and friends are encouraged to be a part of residents’ lives and are encouraged to visit the home.
